If you have a complaint to make against a facility regulated by the Georgia Department of Agriculture Food Safety Division, you may call or email your information to our headquarters office, or to the District Office where the facility is located. The GDA Complaint Coordinator receives consumer concerns involving unsanitary conditions and/or food handling practices for retail and manufactured food facilities in the state, and coordinates the GDA's responsibilities during the investigation of foodborne illness outbreaks.*
The complainant should be prepared to provide a name, contact number and email address, if available. When making a complaint, the complainant will be required to provide the following information:
- Name & address (street, city) of the facility.
- Date of observation(s).
- Date the product(s) were purchase and/or date(s) they were eaten.
- If illness is being reported, when did the symptoms start? How many people are ill in the household?
- Is there any confirmation of illness (documentation is required for "confirmed" illness, including medical, laboratory, doctor's report and/or discharge papers)? If illness is confirmed, the complainant should also contact the health department.
Note: If the food was prepared at a restaurant or food service facility, the complaint will fall under the jurisdiction of the health department for that county in Georgia. If the food was not prepared, handled or packaged on-site at the facility, the complaint should be directed to FDA's Georgia Consumer Complaint Coordinator can be reached at 404-669-4467. If the problem involves meat or poultry products, the complaint should be directed to the U.S. Department of Agriculture, call the USDA hotline at 1-800-535-4555.
*The GDA's consumer complaint line is open during regular business hours, M-F, 8:00 a.m. to 4:30 p.m. If consumers are experiencing symptoms of foodborne illness and are unable to reach the GDA office, they are encouraged to contact the Georgia Department of Public Health's 24/7 Public Health Emergency Line at 1-866-PUB-HEALTH (1-866-782-4584).
